Adult Health Checks

Full Adult Health Assessments are currently being offered through your local doctor free of charge.
The purpose of the assessments are to enable GP’s to undertake an holistic in-depth assessment of the health of a person in the context of their social and physical environment.
The doctor will do a basic assessment of the individuals physical, psychological and social health status and with joint identification with the patient consider the need for any preventative health care, support and/or education to improve functioning and quality of life.
The assessment is open to any Indigenous person’s aged 55 years and over and non-indigenous person’s aged 75 years and over.
You also need to be living in your home only (including retirement village) not in hospital, day hospital, or in a residential aged care facility.
Client’s may have an assessment once per year.

The assessments can be done at the patients home, at the doctors surgery or a combination of both. You may need to schedule two appointments though as they can take approx. 30-40 minutes to complete.
This is a great opportunity for our community to visit your local doctor and have an assessment done to ensure your health is at its best and any problems are diagnosed and preventative measures can be put in place if any issues are discovered.
